Patients are expected to pay co-pays, deductibles, and percentages at the time of visit. All deductibles and percentages will be estimates based on insurance provided amounts. Insurance will be billed for covered services and patients will be responsible for any remaining deductible amount. Cosmetic procedures are payable at time of service.
Payments made by check that are not honored by the issuing bank will incur a returned check fee of $35. Returned check reimbursement payments must be in the form of cash, cashier’s check, certified funds, money order, or credit card.
